- Before the first visit, call the health insurance company to find out whether they require pre-authorization for procedures or treatments. If they do, please call HCI's pre-authorization representative at 801-587-4036.
- If the patient does not have insurance or is concerned about financial issues related to medical treatment, please call 801-587-4036 and ask to speak with an HCI financial counselor.
- Also ask the insurance company whether they require a referral to see HCI clinic doctors. If they do, please contact your primary care doctor and bring the letter or form to your appointment.
- Most insurance companies require the patient to make a copayment before each visit. If a co-payment is required, please bring that amount with you to each appointment. HCI accepts cash, checks, VISA, MasterCard, and American Express.
- Bring the patient's insurance card and prescription card to the appointment. We need the information printed on the cards to bill the insurance company for the services we provide.
